My Health Portfolio is a secure internet site to send the office messages, request non-urgent appointments, change your personal information, change your insurance information, get lab results, request medication refills, get vital signs and other information from office visits, and request information such as immunization and physical forms.

In order to have access to the secure site, ask during your next visit for a login name and password. The password will be generated at the office and then you may change it to one you prefer.
